To achieve wellness in the community by providing an array of primary and behavioral health care, housing, and substance abuse services with dignity and respect.


Central City Integrated Health (CCIH) was designated as a Patient-Centered Medical Home (PCMH) through the National Committee for Quality Assurance in 2017. But our work didn’t start there! CCIH has a 60-year history of providing evidence-based medical and behavioral health treatment and services to Wayne County residents.

At CCIH, patients have access to a care team of providers that focus on all aspects of patient health and overall well-being, including emotional, family, and social concerns. Available services include: Primary and Pediatric Care, Dental Care, Comprehensive Behavioral Health Services, Substance Use Disorder Treatment, Supportive Housing, Community Re-Entry Services, Employment Services, Wellness Services, and Peer Support.

At Central City Integrated Health, our goal is our motto: “Wellness for the Mind, Body, and Community.”
