Q: I’ve heard that surgical treatments for receding gums are very painful. Is there a less invasive procedure?
A: Unfortunately, people will put off having treatment for their gum recession because the standard procedure involves incisions, stitches, and a long healing period. Q: What are same-day crowns?
A: Using CEREC CAD-CAM technology, Dr. Dolores Baran can restore a patient’s tooth with a dental crown or partial crown in just one office visit.
“With CEREC, we take a picture of the patient’s prepped tooth and then design the restoration on the computer screen. Within five to eight minutes, the CEREC machine creates a beautiful and strong porcelain restoration that we bond to the patient’s tooth,”
Dr. Baran explains. “There are no messy impressions and no need to send the patient home wearing a temporary while their permanent restoration is made in a lab.”
CEREC can be used to replace existing crowns or to create multiple restorations in one appointment.

Q: I have an unstable lower denture. Could I be a candidate for mini dental implants?
A: Mini dental implants can provide an excellent solution for the majority of people who suffer pain or frustration as a result of loose-fitting lower dentures.
“These miniature-sized titanium posts stabilize the denture. They’re placed without surgical intervention, requiring only a local anesthetic. Patients experience little to no bleeding and minimal discomfort,” Dr. Frank Ruffino says.
“Another advantage is that we typically can use the patient’s existing denture. We adjust the denture so that it will snap onto the implants. With this procedure, no messy adhesives are needed. Patients will come in for the procedure and walk out wearing a denture that’s securely locked into place until they remove it.”
Patients who are interested in mini dental implants must be evaluated for sufficient bone density, and have an assessment of their existing denture.

Q: Is there a minimally invasive procedure to correct receding gums?
A: With the Chao Pinhole Gum Rejuvenation Technique, receding gums are typically treated with no cutting, no stitches, and virtually no pain. Periodontist Dr. Joseph Nemeth was the first gum specialist in Michigan to be certified in the use of this minimally invasive technique by its originator and developer, Dr. John Chao.
“When gums recede, your teeth look much longer and your smile looks older. This condition also can lead to tooth sensitivity, decay, and bone loss,” Dr. Nemeth explains. “With the Pinhole Technique, we make a small pinhole in the gum above the teeth to be treated. Using special instruments, we loosen up the gum tissue and bring it down over the receded areas, where it should be. This improves the smile tremendously, and prevents further gum and bone recession and loss.”
Dr. Nemeth places collagen material in the treated gum to promote thickness and to add stability. There are variations in the Pinhole Technique. As a gum specialist, Dr. Nemeth has the expertise and experience to know what type of collagen or noncollagen material will work best in each case.
“Another material that we can place through the pinhole is PRF (Platelet Rich Fibrin), a membrane derived from the patient’s own blood. It increases the blood supply in the gums, speeds healing, and further enhances the result,” Dr. Nemeth says.

Q: I’m confused about orthodontic treatment for my 8-year-old. Can she start wearing braces now or should we wait?
A: While the American Association of Orthodontics recommends that children be seen by the age of 7, that doesn’t necessarily mean that’s when treatment should begin.
“For the most part, the optimal age to put braces on the teeth is somewhere between 11 ½ and 12 years developmentally. That’s not chronologic age, as some 9-year-olds already have 12-year molars,” orthodontist Dr. Nick Palmer says.
“There are valid reasons to treat early, such as crossbite and some skeletal issues. However, putting braces on an 8-year-old doesn’t mean they’ll finish their treatment any earlier than a child who starts treatment at age 12. The optimal time to start is one that will give you the best end result in the shortest period of time.”
Dr. Palmer advises parents to become informed consumers so they can make educated decisions.
“If you’re not sure about the treatment recommended by the orthodontist, you should seek a second opinion,” he advises.
