    Q: Should I wait until spring to have cosmetic surgery?

    Q: What is the modern approach to eyelid surgery?

    A: While the old technique for eyelid surgery typically involves the removal of loose skin and fat pouches, often there are many other problems related to the aging of the eye that should be addressed. Aesthetic plastic surgeon Dr. Mune Gowda looks at the framework of the eye, from the eyebrow to the cheek.

    “Changes that can occur around the eye include a loss of volume as the tissues deflate. The orbital bones can thin, and the eyeball will look sunken, with hollow circles under the lower lid. Both the eyebrow and the cheek also can lose volume,” says Dr. Gowda, whose philosophy is to avoid the removal of too much tissue and fat, which can eventually lead to a sunken eye appearance.

    “To create a youthful-looking eye, I take out some skin, as well as fat that is sagging where it should not be. I often reposition fat or add fat through fat grafting. Volume may be needed under the brow, or in the cheek and the hollows under the lower lids, to create a smooth lid/cheek junction,” Dr. Gowda explains. “The muscles might also be tightened to help support the lower eyelids.”

    Dr. Gowda has been doing fat grafts for 15 years. He believes that the patient’s own fat is the best filler.

    “Fat contains a much higher number of stem cells than bone marrow, and it causes no immune problems,” he says. “The skin looks radiant and glowing after fat injections, and fat also improves dark circles.”
