PAIN MANAGEMENT SPECIAL ADVERTISING SECTION
Pain is a tough opponent. It delivers a punch to the head, making you dive under the covers with a severe headache. Pain will pummel you “on the ropes,” zapping your energy and causing bodywide discomfort. Even one simple blow to the back can keep you off the job and make it difficult to sit, stand, or sleep. Luckily, you have someone in your corner who can help you overcome your pain problems. Metro Detroit pain management specialists and other physicians offer a wide range of the latest treatments for both acute and chronic pain, including many noninvasive therapies.
For example, when vertebrae in the spine are moved out of position from an accident, a chiropractic physician can perform a hands-on adjustment to realign the vertebrae, relieving pressure on pinched nerves. Pilates and postural exercises are drug-free, conservative therapies for improving an aching back. Pain that keeps you off your feet may be treated easily with custom-made orthotics. Medicated creams and noninvasive lasers will deliver relief directly to your sore spots.
When throbbing pain takes root in your mouth, you don’t want your treatment to hurt, too. The fact is, dental professionals are helping patients feel comfortable and totally relaxed so that a root canal or other treatment is no big deal.
Talk with your physician when pain strikes. Learn about your diagnosis and the available treatment options so that you can make a comeback and enjoy a better quality of life.

Q: Why do I need to visit a chiropractor if I can pop my own neck and back?
A: If you’re constantly popping or cracking your back and neck, stop! Depending upon your specific spinal health, auto manipulation or adjusting yourself may be harmful. Most people overuse this technique, causing hyper-mobility — too much movement of the joint. This can lead to headaches and chronic lower back pain.
Professional chiropractic adjustments, on the other hand, keep normal inter-segmental movement in the entire spine. Breaking the habit of popping or cracking isn’t easy, but it’s the first step to better spinal health.

Q: How can posture alignment improve my backache?
A: Poor posture is the primary cause of neck and back problems, says physical therapist and Pilates instructor Ron Jegadeesh.
“The first thing people notice when they do Pilates is that their posture and alignment have changed. They have a sense of power, grace, elegance of movement, and self-confidence. As the sessions progress, they notice improved flexibility and strength,” he explains. “I also teach postural exercises the client can do at home or at work. I ask them to make a habit of keeping their body in good alignment as they go about with their daily activities. They learn to sit taller, stand taller, and walk taller.”

Q: I have a chronic pain issue. Will I feel comfortable during my dental visit?
A: “Some people who have discomfort in one part of their body might not think that they can tolerate sitting in a dental chair, so they will postpone their procedures. Unfortunately, neglecting to treat dental problems early can often lead to greater problems,” says Dr. Craig Goldin of the award-winning Cosmetic Dentistry Institute, where patient comfort is a priority.
Dr. Goldin and his wife, Dr. Marcy Goldin, are dedicated to providing superlative dental care that is efficient and high-tech, as well as relaxing and painless. Their beautiful office is nearly 6,000 square feet of the most advanced dental technology in a soothing, spa-like environment.
“Imagine you’re wrapped in warm blankets and lying in a comfortable position on top of a soft Tempur-Pedic cushion. Your choice of music is playing in your headphones, and you’re completely calm and comfortable,” says Dr. Marcy Goldin, an adjunct clinical lecturer at the University of Michigan School of Dentistry. “With the option of sedation dentistry, you can be totally relaxed while having multiple dental treatments completed — often in as little as one or two visits.”
“Sedation dentistry allows us to remove the fear from patients who experience any type of dental anxiety,” adds Dr. Craig Goldin, who has specialized training in IV sedation. “Most people have little or no memory of their procedures, and are comfortable and pain-free throughout their entire visit.”
Delivering the highest level of patient care and comfort, the Cosmetic Dentistry Institute provides every phase of general, cosmetic, implant, and sedation dentistry in one office, for your convenience.

Q: Why should older people be cautious when taking over-the-counter pain relievers?
A: When using over-the-counter pain medications, it’s important for older people to carefully read directions to avoid the risks of serious side effects. For example, exceeding the recommended maximum dosage of 4,000 milligrams of acetaminophen per day may lead to liver toxicity. With the availability of 500-milligram over-the-counter tablets, and due to the fact that many people also use prescription pain medications containing acetaminophen, it’s possible to accidentally overdose.
Seniors who take low-dose aspirin to prevent blood clots and also use aspirin for pain may be at risk of gastric bleeding. Older adults are encouraged to talk with their physician and pharmacist about all of their medications and supplements, to prevent complications.

Elite Podiatry
Q: What types of noninvasive treatments can help relieve pain?
A: The specialists at Elite Podiatry often recommend a personalized course of treatment using nonsurgical therapies, including custom-made medicated creams, for specific pain issues.
“The advantage of using creams is that the medicine is delivered through the skin directly to the site where it hurts,” says Dr. Alexsey Kheynson, D.P.M. “There are fewer side effects with medicated creams and less chance of dependency issues than with long-term use of oral medications.”
Custom-made orthotics composed of strong and lightweight carbon fi ber can help prevent fatigue and injury. In addition to using them in everyday shoes, orthotics can be placed in cold-weather footwear like snow boots, skates, and ski boots.
Bracing is another option of noninvasive treatment offered by Elite Podiatry. “In icy weather, people slip and often will sprain an ankle or twist their foot. Bracing helps to provide support and may prevent the need for surgery,” Dr. Kheynson says. “Bracing can also be helpful for people who have a longstanding injury that didn’t heal correctly, and is now giving discomfort or lack of stability. Most braces also have a foot plate where we can insert an orthotic.”
FDA-approved MLS Laser Therapy painlessly and effectively treats many painful conditions without any negative side effects.
“It’s nothing less than a miracle for reducing infl ammation and for rapid relief of pain,” Dr. Kheynson says. “The laser can help to speed up healing of tendon and ligament ruptures. It works phenomenally for people with neurological pain, especially those with diabetic neuropathy.”

Q: I have pain from the neck to the low back since I had an auto accident. Can chiropractic help me?
A: Yes. The key to treating any condition is to know the mechanism of injury. When someone is involved in an accident, vertebrae shift out of place, affecting the discs, ligaments, and muscles. This can lead to sprain or strain injuries, or nerves being pinched. Pain can come in the form of muscle spasms, tingling and numbness in the lower and upper extremities, or it can be felt as a dull ache or burning.
We use digital X-rays to accurately identify the cause of pain. Then we deliver an adjustment to help alleviate the pain by aligning the spine, which takes pressure off the nerves. While medications may temporarily relieve pain, chiropractic adjustments can correct the problem that causes the pain.

Michigan Head & Spine Institute
Q: What does a physiatrist do to relieve neck and back pain?
A: The physiatrists of the Michigan Head & Spine Institute (MHSI) specialize in the nonsurgical evaluation and management of back and neck pain. Their goal is to improve a patient’s functional skills and, thus, their quality of life. Central to this is the elimination of pain, which results in minimizing the use of medications. Physiatrists also diagnose various neuromuscular disorders with electrodiagnostics, or tests that provide information on how the nervous system functions.
Using conservative, nonsurgical tools and minimally invasive treatments, pain management, and exercise-based rehabilitation therapy, physiatrists treat various musculoskeletal (muscles and bones) and neurological (nervous system) disorders including arthritis, back pain, work- and sports-related injuries, brain or spinal cord injuries, post-stroke treatment, and brain and spine injuries. They also work with amputees to address their specific challenges and help them to function better in everyday activities.
MHSI is one of the largest neurosurgery/physiatry medical practices in the country, with several locations in southeast Michigan. If you’re seeking conservative and minimally invasive techniques to relieve pain, you owe it to yourself to make an appointment with a physiatrist, or physical medicine and rehabilitation (PM&R) specialist. While the American Medical Association has recognized physiatrists since 1946, their role is often misunderstood — except by those who have benefited from their expertise. At MHSI, the physiatrist makes every effort to find a solution to your pain that considers your lifestyle, your level of activity, and your desired quality of life.

Q: Will I feel comfortable during a root canal procedure?
A: Everyone who comes into our office has heard stories or jokes about the pain associated with root canals. The fact is, a recent survey conducted by the American Association of Endodontists found that people who actually experienced root canal therapy were three times more likely to describe their visit as painless than those who have never had the procedure. Our goal is to make patients feel as comfortable as possible in a friendly and supportive environment. With our state-of-the-art equipment and the proper use of anesthesia and post-treatment medication, be assured that your root canal experience can also be pain-free.

Q: My doctor said I need knee surgery. Is there an alternative treatment that doesn’t require a painful and lengthy rehabilitation?
A: Stem cell therapy is a breakthrough treatment for people who have chronic degenerative knee pain. We remove stem cells from the patient’s bone marrow and then inject those cells into the damaged knee joint to regrow articular cartilage that has worn down.
This brief outpatient procedure is safe, essentially pain-free, and requires no or little downtime. Patients have reported quick and progressive improvement. We were the first recipients of our treatment and have personally experienced the benefits.
Stem cell therapy is a one-time procedure for potentially lifelong cartilage replacement. We’re the first in Michigan to offer this therapy, and we’re eager to begin treatment in other joints of the body.

ANA Pain Management P.C.
Anand C. Thakur, M.D.
Q: What do you consider the role of pain management?
A: The key is to first establish a diagnosis with a careful history and physical exam. Most times, common sources of pain are effectively managed with medications prescribed by your primary care physician. When this doesn’t work, you need a specialist in the medical and interventional treatment of pain, notably a board-certified and fellowship-trained pain physician. Care involves a biopsychosocial treatment approach with medical management, physical therapy, and minimally invasive treatments.
It’s important to explain to patients exactly what their diagnosis is and describe the treatment options that are available. We have a defined functional goal for our patients, and we give them options to reach that goal. We offer a wide variety of treatment options and comprehensive care for all of your pain needs.
