PLASTIC & COSMETIC SURGERY – SPECIAL ADVERTISING SECTION
Life is all about choices. Regular or decaf? Pizza or pasta? Comedy or action thriller?
Most choices are easy to make, but when you’re looking for a treatment to “turn back the clock” or to give you a more attractive appearance, the number of cosmetic options can be daunting.
Nonsurgical, minimally invasive procedures offer the benefits of little to no downtime, they’re less expensive, and they’re good alternatives for people who are apprehensive about surgery. Botox and soft-tissue fillers can take years off your face in almost an instant. Innovative tools — from fractional lasers that rejuvenate damaged skin to ultrasound-based therapies that tighten, contour and lift — deliver great results without surgery.
Now, while your best friend may look fabulous after a “liquid face-lift,” a nonsurgical treatment may not meet your individual needs. Based on your goals and expectations, a surgical eye-lift, a brow lift and/or a face-lift may be a more suitable choice, giving you a better and longer-lasting outcome. Today’s surgeons are using the latest minimally invasive techniques to minimize scarring, reduce downtime, and enhance the results of surgery.
What’s the best approach for your cosmetic concern? To make an educated decision, book a consultation, bring along a checklist of questions, and ask to see before-and-after photos. You can get started right now, by reading answers to some common questions.
Dr. Myra Danish, M.D., F.A.C.S.
What Can Be Done In An Hour Or Less To Rejuvenate My Facial Appearance?
“Surgical procedures that take about an hour include a brow-lift for brows that are drooping below the bony rim of the eye, and upper and lower eyelid-lifts for droopy eyelids or bags under the eyes,” says facial plastic surgeon Dr. Myra Danish, M.D., F.A.C.S. “In less than an hour we can also do cheek augmentation or chin augmentation, which typically involves implants to improve the overall profile of the face.”
Dr. Danish is known for pioneering the Lunchtime Lift™, a mid-face lift that lifts the cheeks, the lower eyelids, and the jowls to restore the natural curves and youthful fullness of the face. She also performs the mini-lift, which is a short version of a face-lift focusing on the jawline and neck.
“Recovery takes about a week. There is little pain involved — just some swelling and bruising,” Dr. Danish explains. “All of these surgeries can be performed in an hour and will restore your facial anatomy to where it was over a decade ago.”
Nonsurgical skin treatments — including laser photo facials, glycolic peels and deeper peels — can leave the skin looking brighter and more refreshed in very little time. With the injectable Lunchtime Lift™, Dr. Danish uses a range of FDA-approved injectables to help her patients look more than 10 years younger in less than an hour.
“Patients experience little to no downtime and see immediate results,” Dr. Danish points out. “They look rejuvenated and natural, not like they’ve had work done.”

Miller Vein
What Does Interventional Radiology Have To Do With Varicose Or Spider Veins?
Vein treatments require ultrasound imaging for diagnosis and technical catheter skills for treatment — both unique abilities of interventional radiologists. These highly trained physicians specialize in nonsurgical procedures and are experts in performing complex treatments through tiny incisions in the skin using imaging guidance.
Considered a pioneer in his field by his colleagues and voted one of Hour magazine’s “Top Docs,” Jeffrey H. Miller, M.D., has been performing complex image-guided endovascular procedures since 1994. In September of 2006, Dr. Miller founded Miller Vein, with a vision of recruiting an exclusive team of specialists who are leaders in the treatment of vein disease.
At the top of his list was Steven K. Wang, M.D., the first physician to bring minimally invasive endovenous treatments for varicose veins to William Beaumont Hospital. Dr. Wang’s expertise in the treatment of complex vascular diseases is well-known to other physicians.
Michael E. Lulenski, M.D., trained under the tutelage of Dr. Miller during his residency. In 2004, Dr. Lulenski was one of the founders of Legacy Vein Clinic, located in South Bend, Ind. He was the first in that region to perform endovenous thermal ablation to treat varicose veins.
At Miller Vein, this world-class team of board-certified interventional radiologists specializes only in venous disorders, and they use the latest state-of-the-art, nonsurgical VeinErase™ treatments, which are virtually pain-free and require no downtime. Patients are walking immediately after their procedure and are thrilled with their results. Miller Vein now offers four convenient locations, where the most knowledgeable, experienced, and dedicated physicians are there to give you life-changing results: Legs Like New!

When Should I Consider Plastic Surgery Over Nonsurgical Alternatives?
Nonsurgical alternatives to plastic surgery are extremely popular, as these options can offer great results and improvements without the expense, downtime, and apprehension associated with going under the knife. But for some people, there is no amount of Botox, fillers, or laser procedures that can repair the underlying issues of muscle laxity and sagging skin that come with age, or the appearance of certain features that are simply due to genetics. In such cases, surgery may be the only option to provide the lasting and desired results that nonsurgical procedures simply cannot. Dr. Charles M. Boyd recommends surgery when the primary area of concern is due to the effects of gravity — as in eyelid, neck, and face-lift procedures. Another example of something that calls for surgery is when a patient wants permanent improvement in the appearance of their nose. As with any procedure, the major consideration for surgery should be based on an analysis of the best way to treat the underlying condition.

What Are The Benefits Of Body-Contouring After Massive Weight Loss?
Body-contouring procedures remove significant amounts of loose, hanging skin and tissue, providing the patient with an improved body shape, enhanced self-esteem, better hygiene, and increased comfort. For patient John Ross, body-contouring is the culmination of his weight loss journey.
“I’ve lost over 270 pounds. It was a lot of hard work to get here, and the surgery puts a finality on what you’ve done,” says Ross, who had 20 pounds of excess skin removed from his trunk and arms.
“The incisions are so small, they’re almost invisible. Dr. Ellen is not only a skilled surgeon; she has the gift of compassion and caring.”

Mune Gowda, M.D., F.A.C.S.
What Is The Modern Approach To Eyelid Surgery?
While the old technique for eyelid surgery typically involves the removal of loose skin and fat pouches, there are many other problems related to the aging of the eye that should be addressed. Aesthetic plastic surgeon Dr. Mune Gowda looks at the framework of the eye, from the eyebrow to the cheek.
“Changes that can occur around the eye include a loss of volume as the tissues deflate. The orbital bones can thin, and the eyeball will look sunken, with a hallow circle under the lower lid. Both the eyebrow and the cheek also can lose volume,” explains Dr. Gowda, whose philosophy is to avoid the removal of too much tissue and fat, which can eventually make the eyes look sunken.
“To create a youthful-looking eye, I take out some skin as well as fat that is sagging where it should not be. I often reposition fat, or add fat through fat grafting. Volume may be needed under the brow, or in the cheek and the hollows under the lower lids, to create a smooth lid/cheek junction,” Dr. Gowda says. “The muscles might also be tightened, to help support the lower eyelids.”
Dr. Gowda has been doing fat grafts for 15 years. He believes that the patient’s own fat is the best filler.
“Fat contains a much higher number of stem cells than bone marrow, and it causes no immune problems,” he says. “The skin looks radiant and glowing after fat injections, and fat also improves dark circles.”

Why Can’t I Lose Weight On My Own?
“Most people make an effort to eat healthy foods and get some exercise, but losing weight is more complicated than that,” says Dr. Gina Randhawa, a bariatric/internal medicine physician who offers personalized medical weight loss plans that focus on diet, nutrition, exercise, and simple lifestyle changes.
“Weight loss is more than just counting calories. There’s a difference between 100 fat calories and 100 protein calories. There’s a right way of exercising and a wrong way of exercising,” she says. “Putting the patient on a more structured routine is easier for them to follow, and they see results they didn’t see before. I’ve never had a patient who did not lose weight.”
Patients also learn valuable information that will help them maintain a healthy weight in the future.
“As much as we want to do it on our own, it always helps if you’re losing weight under supervision and there’s some accountability involved,”Dr. Randhawa says.

What Is A ‘Mommy Makeover’?
Breast augmentation, breast-lifts, and tummy tucks are the common cosmetic surgeries that help new moms improve their post-pregnancy figure.
“We’re addressing problems that the patient cannot physically change on her own,” says board-certified plastic surgeon Dr. Daniel Sherbert. “You can’t make the excess skin and fatty tissue disappear from your abdomen, or reverse changes to the breasts that come from pregnancy and breastfeeding. Through surgery, we can help patients get back to where they used to be before having children. They can feel comfortable wearing fitted clothing or a bathing suit again, and feel more like themselves.”
When more than one surgery is recommended, busy moms typically choose to have all of the procedures that can safely be completed together done at one time.
Dr. Sherbert says many busy professionals are coming in for cosmetic procedures to keep in step with younger colleagues. Upper and lower eyelid-lifts can refresh tired-looking eyes, while Botox smooths “angry,” furrowed brows.

What Techniques Can Enhance The Results Of Breast Reduction?
Having performed nearly 3,500 breast-reduction procedures, board-certified plastic surgeon Dr. Ahmet Karaca is one of few surgeons in the area who performs a seamless breast reduction without the need for a horizontal scar. The surgery involves incisions that go around the areola, then vertically down to the breast fold in a “lollipop”-shaped pattern.
“The nipple rests on a peninsula of tissue, with its blood supply intact. This method allows us to more easily reposition the nipples and to remove more tissue from the breast,” Dr. Karaca explains. “With this approach there’s less occurrence of ‘bottoming out,’ or eventual drooping of the breast.”
To prevent scarring, Dr. Karaca asks the patient to tan before surgery. Then, for two months after surgery, one-inch-wide paper tape is placed on the scars. At the end of two months, the patient tans again.
“Tanning has a negative effect on the collagen deposit,” Dr. Karaca says. “Combining these techniques can greatly minimize scarring.”
