A: The rotator cuff is actually a group of four muscles and tendons that surround the shoulder joint. Its primary function is to strengthen and stabilize the shoulder. Above the rotator cuff is the bursa — a sac of tissue that covers and protects the rotator cuff when it comes into close contact with the bones around the shoulder. When the rotator cuff is injured, it can cause inflammation of the bursa, a condition known as bursitis. The result is pain, weakness, and decreased range of motion in the shoulder — particularly when it comes to lifting the arm. This can make even the simplest of daily activities difficult.
Rotator cuff injuries are more common in men, and they’re often caused by repetitive activities such as those found in sports or physical jobs. But aging and tissue degeneration could also be culprits. Whatever the reason, there’s no need to rush to a surgeon.
“Both physical therapy and Pilates go a long way toward improving the function of the muscles around the shoulder,” says Ron Jegadeesh, physical therapist, Pilates instructor, and owner of Pilates Fitness & Physical Therapy Center. “Rather than simply strengthening the large muscles around the shoulder, they target the smaller but equally important muscles. In strengthening these muscles, the exercises and movements of Pilates and physical therapy compensate for damaged tendons. In fact, they can even improve the mechanics of the shoulder joint. Delcomyn notes that many people don’t stretch enough before they play a round of golf. “We show our patients stretching exercises that can improve their flexibility. A 10-15 minute warmup can help a lot,” he says. “It can be even more beneficial to stretch after your golf game when your body is loose and limber.
“If you feel pain after a weekend of golf, don’t ignore it,” Delcomyn adds. “We can treat what’s hurting and tell you what movements caused the pain to prevent future injuries and get you back on the course.”
Q: What can I do to help relieve my back pain?
A: While there is no substitute for seeking professional medical advice, there are things you can do at home to improve your symptoms. Lifestyle changes are among the most important things you can do to help your condition. The first thing is to work on your posture, remembering to sit and stand straight while flexing your abdominal muscles. This means no slouching! Secondly, change your sleep patterns by finding the best position for pain relief, which varies from person to person and condition to condition. Try to get six to eight hours of sleep depending on your current patterns. Third, improve your diet and eating times. Weight loss, especially in the abdominal area, will strengthen the overstretched back ligaments, muscles, and tendons that worsen posture and back pain. Fourth, and certainly not least, is exercise. There are many types of exercises you may choose from depending on your goals, but the single most important exercise you can do will be one that focuses on core conditioning. Properly coordinated core muscles stabilize the spine and create an incredibly firm base of support for practically all movements. Q: My upper eyelids appear to be drooping. What type of procedure can fix this problem?
A: There are three reasons why you might think your eyelids are drooping:
- Do your upper eyelids droop over your vision like a window shade? The levator muscle inside the eyelid lifts the eyelid up and back to open your eye. If the levator muscle is loose, detached, unhealthy, or damaged, you have ptosis, which is a true drooping of the upper eyelid. In this situation, we repair or tighten the muscle.
- Between the eyelid and the eyebrow, you may have excess skin and/or fatty tissue that droops over your eyelids and sometimes over the eyelashes. The extra skin and tissue are removed with a blepharoplasty — a procedure that people often call “getting their eyelids lifted,” although nothing is actually lifted.
- If your eyebrow and forehead skin come down over the eyelids or over the boney rim of the eye sockets, that is called brow and forehead ptosis (drooping). The treatment is a forehead/ brow lift.
Each individual patient can have any one or more of these conditions. It’s really important that the contribution of each of these factors is identified. Your individual eyelid anatomy and the changes that have occurred to your eyelid with aging or genetics should be carefully considered before deciding on what procedures are appropriate for you.
